WE DO NOT DEAL WITH CRITICAL CARE EMERGENCIES | CONTINUAL CARE ONLY DEALS WITH PRE PLANNED HOME HEALTH CARE SERVICES

Terms and Conditions/ Privacy Policy of Continual Care’s, Pre/Post Hospitalization and Elderly Care Services which must be read thoroughly by the Patient’s/Elderly Family Members Son/Daughter or any other authorized person (Can be any other relation of the Patient/ Elderly Family Member) for understanding the same and signing on the “Service Contract Document” before availing services which are mentioned below:-

  1. Continual Care having its Registered Office at 13B, lake Temple Road, Kolkata- 700 029 has valid Trade license in the name of the Firm. In future they may open other branches if the company decides so.
  2. Continual Care provides Nursing supervision and care and Para- Medical support as per the Medical Advice of the patients treating Doctor only (Primary Treating Doctor or House Physician ).
  3. Continual Care will provide two Categories of Nursing- Basic Nursing Care and Senior Nursing/ Technicians as per the need and advice of the Doctor.
  4. The Nursing Staff should not be requested or forced to give any treatment advice by you or any other family member. In such case Continual Care will not take any responsibility for the outcome of your Patient/ Elderly Family Member.
  5. Only Senior Nurses / Technicians will perform procedures like IV Channel/ IV Antibiotic/ IV fluid or any other Injection/ Catheter insertion/ Tracheostomy Tube dressing/ PEG Tube Dressing/ Major wound dressing/ Ryle’s Tube insertion as per the treating Doctor’s advice only.
  6. Junior Nurses will give basic Nursing Services like- Feeding Medicines, Ryle’s Tube feeding/ Measuring Urine output/ Body temperature/ Blood Pressure Monitoring, Blood Oxygen Monitoring ( SpO2), Oxygen administration as advised by your treating Doctor, Diaper Changing only. “NURSING STAFF SHOULD NOT BE FORCED TO DISPOSE STOOL AT ANY POINT OF TIME”. They will also perform small wound dressing only.
  7. The Nursing Staff will give periodic updates of your Patient’s Overall condition to the treating Doctor and to you.
  8. Before engaging Continual Care as a Service provider The Patient’s / Elderly Family Members current and previous “Medical Records” must be disclosed to the Supervising Staff of Continual Care. Medical History should not be concealed at any point of time. The Complete Medical history must be disclosed to Continual Care 24 Hrs before.
  9. Before appointing Continual Care to provide Home Healthcare Services, the owner of Continual Care or its authorized representative will pay an inspection visit to the Patient/ Elderly Family Members residence and a separate charge will be applicable for the same depending upon the distance from Continual Care’s office.
  10. Work Place safety and due respect of the Nurses must be ensured by you. Nurses must feel safe while discharging their duties. If any untoward incident happens to a Nurse on duty Continual care will have the right to initiate legal action as per the Law of The land.
  11. If Death happens to a Patient/ elderly Family Member the Nurse on duty/ Continual Care should not be held responsible for the same as Nurses are care givers only.
  12. The organization will not be held responsible for any loss or damage of any article at your home. You are requested to keep Money/ Valuables at safe custody only.
  13. If at any point of time from the Patient/Elderly family Member will face any attitude problem/ Behavior of the Care giving Nurse the same must be intimated to Continual Care within Office Working Hours– Morning 9am till Evening 8pm. Within a reasonable time frame the appointed Nurse will be removed from duty and a new Nurse will be appointed. Though before appointing any Nursing Staff Continual Care and Its associates ensures that a nurse appointed for Care giving is modest and gentle with her behavior.
  14. Son/Daughter or any other Relative who will avail homecare Services from Continual Care for their Patient/ Elderly family member must have a local guardian- Either someone from the family/ Friends who will function as the key point of contact for any medical exigency/ Day to Day updates or if a Patient/ Elderly family member would require Hospitalization. Continual care will not take independent decision or select hospital for such an admission. Continual Care will arrange Ambulance/ Transport for shifting the Patient within morning 9am till 8pm only.
  15. Staff of Continual Care will not handle any Medical Emergency. They will only inform the Contact Person / you if the health condition of your patient deteriorates and demands hospitalization.
  16. A Minimum of 10 days Home  Nursing Service and for Medical equipments 15 days rental  must be booked in advance and payment shall be made either through GPay/ Phone Pay or Bank transfer only. Continual Care does not accept cheque payment. In case of death of a Patient for Medical Equipment rental charge will be non-refundable. Charges accepted for Nursing Care service will be partially returned after adjustment of incidental expenses.
  17. You are hereby requested not to enter into direct contract with any of our staff. If such incident is noted Continual Care will have the right to initiate legal action as governed by Law.
  18. Continual Care will not undertake any responsibility to arrange death certificate in case your treating doctor is unavailable or there is any unnatural or accidental death.
  19.  Continual Care will extend supervising level service from Morning 9 am to 8pm (Office working hour). Any other Service related issue will be resolved next day after 9am
  20. For safety of your Patient’s health monitoring, you are requested to mandatorily engage your house physician to periodically pay visits at your home.
  21. A Care giving Nurse will work on rotational duty basis (12 hours shift) until and unless, due to unavoidable circumstances She is will have to Continue her duty. For which additional pay as per pre- fixed rate will be charged.
  22. Mode adjustments of Medical Equipments like Oxygen Concentrator, Bi-pap, C-pap will be done by Senior Nurse / technician as per treating doctors guidelines only.
  23. External influencing factors like “Natural Calamity, Road Obstruction. Bandh, Transport issue etc. may affects Nurse joining duty on time. However   interim arrangement will be

made as soon as possible to ensure care to your patient. Else the Nurse on duty will continue her Duty till the situation becomes normal.

Special Note:- Please Read the Terms and Conditions/ Privacy Policy as mentioned above and then sign on the Contract letter (Full Signature mentioning Date and Time) and your Relationship with the Patient/ Elderly Family Member.
